KentuckyOne Health Pharmacy Plus and Pharmacy Plus Specialty Closure Information

As our organization continues to adapt to changes in the way people access care and manage their health, we have made the decision to close Pharmacy Plus and Pharmacy Plus Specialty retail pharmacy locations at our facilities, effective June 21, 2019. While we will no longer operate the retail pharmacy locations open to the general community, we will continue to operate pharmacy services to dispense medications and medical supplies to our team delivering patient care at our facilities. 

We apologize for any inconvenience this will bring to patients currently using these pharmacy services. We are committed to making the transition as smooth as possible with the following information on the transition of operations and venues for prescriptions:

The specific pharmacy locations impacted are:

Retail Pharmacy locations:

  • Pharmacy Plus at Jewish Hospital/Frazier Rehab Institute
  • Pharmacy Plus at Sts. Mary and Elizabeth Hospital
  • Pharmacy Plus Specialty at Commerce Crossings

The following provides information on key elements of this change to pharmacy services. In addition, Pharmacy Plus and Pharmacy Plus Specialty team members are available to discuss specific questions and concerns by calling 1-844-773-6145, Monday through Friday, 9:00am to 4:00pm EST.

Changes to Prescription Access & Pick-up Location

  • We recommend that all current Pharmacy Plus and Pharmacy Plus specialty patients review their prescriptions and confirm the new default pharmacy location (see below) or make a change to their pharmacy location by Friday, June 7.
  • Patients have the option of selecting a pharmacy of their choice to fulfill prescriptions, based on the type of medication received. If no action is taken to change pharmacy, existing prescriptions will be transferred to a new pharmacy provider, based on the specific type of medication prescribed.

  • When receiving new or updating existing prescriptions, we recommend that you confirm with your physician and care team the new location of your pharmacy provider.
